All claims must be submitted to an insurance plan (both personal and spousal primary plans) before being submitted against a Spending Account for reimbursement.

Typically a primary insurance provider will only cover a portion of the expense that was submitted. League can reimburse any uncovered portion of your eligible expenses under your Health and/or Lifestyle Spending Account. When submitting a claim against your Spending Account, please ensure you include a claim statement form from your primary insurance provider. This document should list the following: provider name, total claim amount, the amount that was reimbursed, as well as the remaining, unpaid balance.

Note: A Spending Account is not considered insurance. When you are submitting a claim to an insurance provider, you should not indicate that the Spending Account is an insured plan. A Spending Account is a supplementary benefit and should be used after all other forms of insurance have been exhausted.
